Important Notices for Students Expected to Graduate in this Semester (114-2)

I. Eligibility and Course Registration

Please verify whether you meet the graduation requirements (complete the Academic Ethics course and pass at least 30 credits) and add the Master's Thesis course (Course Code: MPH7999) during the course selection/add-drop period of the current semester.

II. Outstanding Document Submission

Please submit any remaining files from your internship:

1. Assignments due during the internship period:

   • (1) Practicum/Internship Midterm Report [Electronic File] and Practicum/Internship Final Written Report [Electronic File] (For layouts/formatting, see Appendix 1).

   • (2) Two pieces of professional work deliverables/products [Electronic File] and the Results/Deliverables Description [Electronic File].

2. Internship Site Evaluation Form (Appendix 2): Please request your external internship site supervisor to return this form to the Degree Program Office via email or hard copy.

3. Practicum/Internship Core Competency Evaluation Form: Signed by your institutional (on-campus) advisor.

III. Application for the Degree Examination (Oral Defense)

Please fill out the following documents: (Note: These documents must be signed/stamped by your thesis advisor before being submitted to the Degree Program Office. The office will then forward them to the Program Director for final review and signature.)

1. Degree Examination Application Form: Please apply through the NTU Office of Academic Affairs "Graduate Students Degree Examination Application System". Print the completed form out and have your thesis advisor sign it.

2. Graduation Credit and Grade Audit Sheet (Appendix 3): To be filled out by the student.

3. Thesis Advisor Recommendation Letter (Appendix 4)

4. Students who have not completed the Academic Ethics course are not permitted to hold an oral defense.

5. Draft of the Practicum/Internship Final Report (Master's Thesis): The research topic shall be mutually decided by the institutional advisor, the external supervisor, and the student. It can be an extension of the internship project or a standard research-oriented thesis. [Electronic File in WORD format; used by the program office for plagiarism/originality check.]

6. Master's Degree Examination Review Fee, Travel Expense, and Thesis Advising Fee Ledger [Electronic File] (Appendix 5):

   Please fill out details thoroughly: examiner's current institution/department/job title, ID number, full registered residential address (including neighborhood/Lin), 3+3 digit zip code, and post office or bank account number (including branch name). [If the account is not with the Post Office, Hua Nan Bank, or E.Sun Bank, a photocopy of the bank passbook must be attached.]

   • ※ On-campus/NTU faculty do not need to provide personal data, but the payment amounts must be specified.

   • Note: If a degree examination committee member does not hold a teaching position of Assistant Professor or above, their qualifications must be reported to and approved by the Degree Program Committee meeting. Please refer to the National Taiwan University Enforcement Rules for Doctoral and Master's Degree Examinations (Article 5, Paragraph 3).

9. Parking Application Form (Appendix 14): If committee members need parking space at the College of Public Health Parking Lot (No. 19, Xuzhou Road), please fill out the form and send it back [Electronic File] at least 7 days prior to the oral defense.

10. If committee members require an official appointment letter, please explicitly state this in your email when sending back the electronic files.

⏱️ ---------- Documents 5–9 above must be submitted electronically 7 days prior to your oral defense. (Late submissions may cause delay as the program office requires time for thesis quality assurance checks, university review submissions, and processing committee fees.) ----------

11. Letter of Approval from the Oral Defense Committee (Copy): Keep the original copy for yourself. (Appendix 8)

13. Degree Examination Core Competency Evaluation Form (Appendix 10): Only print the section corresponding to your specialization group.

14. Disbursement Receipt Ledger: Please pick this up at the Degree Program Office on the day of your oral defense for committee members to sign.

15. Application for Delayed Public Disclosure of Thesis (Effective from the 2025/114 Academic Year onward): If your thesis involves confidential data, patents, or legal restrictions that prevent immediate publication, you must provide relevant materials to the examination committee during the oral defense. Fill out the "National Taiwan University Application for Delayed Public Disclosure of Thesis Form" and have all committee members review and sign it. Every application must attach an original copy of the review form and original supporting documents. If no official supporting document is available yet, a signed declaration statement outlining the reasons for the delay may be submitted. Please refer to the NTU Library website for detailed regulations.

IV. Important Timeline and Deadlines for the Degree Examination

Please make sure to complete each procedure before the specified deadlines:

• ※ Please be sure to allocate and buffer enough time for the NTU Library's review process.

• ※ The Office of Academic Affairs will audit the school leaving system on the morning of August 26. If your status for the "Library" step shows "Completed" (已辦畢), you will graduate as a 114-2 student. If it shows "Incomplete" (未辦畢), you will be required to register and pay tuition for the 115-1 semester. (Those who have reached their maximum duration of study will be dismissed from the university).

VI. Miscellaneous & FAQs

1. Master's Thesis - Practicum/Internship Final Report: This refers to your MPH thesis, which can either be an extension of your internship work or a standard research-oriented thesis. For formatting regulations, please refer to the National Taiwan University Format Guidelines for Doctoral and Master's Theses and Dissertations (Appendix 12).

2. Q & A

   • Q: If I registered for the "Master's Thesis" course but cannot apply for the degree examination in time, do I need to apply for a "Course Withdrawal (Drop)" for the thesis course?

   • A: No. Registering for the "Master's Thesis" or "Doctoral Dissertation" course simply indicates that you are in the process of writing. It is perfectly fine if you do not apply for an oral defense this semester; there is no need to withdraw from the course. If you have already applied for the oral defense but cannot finish the thesis, you only need to apply to cancel the degree examination. (For the Degree Examination Cancellation Form, see Appendix 13).

   • [According to the NTU Enforcement Rules for Doctoral and Master's Degree Examinations: Graduate students who have applied for the degree examination but cannot hold it during the semester for any reason must apply to the university to cancel the examination before the semester-end date specified on the school calendar. Those who fail to cancel and do not hold the examination by the deadline will receive a failing grade for that attempt.]

• Please allocate sufficient time for the library to review your electronic thesis format and for printing the hard copy. You must complete the hard copy submission by the deadline (August 11). Students who fail to submit their thesis on time and have not reached their maximum study duration must register for the following semester. Those who have reached the study limit and fail to submit their thesis will be dismissed.

IX. Poster Exhibition

The Degree Program will hold a Poster Exhibition at the end of the semester (June 1–5, 2026). The content may showcase either your practicum results or research outcomes. Graduating students are highly encouraged to participate. (Attendance is mandatory for students who enrolled in the R12 academic year).
