Guidelines for Students Applying for Internships in the 114-2 Semester
Please pay close attention to the following instructions if you wish to apply for an internship in the 114-2 semester:
★ Students may complete their internship at their current workplace (provided the internship tasks differ from their routine job responsibilities or serve as an extension of their original professional field). If necessary, students may also request their institutional (on-campus) advisor to arrange an internship site.
I. Prerequisites
Before submitting an internship application, ensure that you have already completed or are expected to complete all core required courses during the current semester of your internship application.

Core Required Courses: Epidemiology and Biostatistics, Health Policy Management and Behavioral Sciences, Environmental and Occupational Health, Public Health: Perspectives and Outlooks, Public Health Research Methods.
II. Required Application Documents and Procedures

By March 13, 2026 (115): Confirm your institutional advisor and submit the "Thesis/Dissertation Advising Agreement Form" (as shown in Appendix 1).

Confirm your external (off-campus) internship site supervisor, then fill out the "Practicum/Internship Application Form" (as shown in Appendix 2).
⚠️ Note: Please submit a hard copy of the Internship Application Form to the Degree Program Office. Once reviewed and approved by the Curriculum Committee, your proposal oral defense can be scheduled.

Application Review Results and Subsequent Steps:
The review results are categorized into: Approved, Revision Required for Re-evaluation, and Not Approved.

A. Approved: You may proceed to schedule your Practicum/Internship Proposal Oral Defense.

B. Revision Required for Re-evaluation: You must email the electronic file of your revised application to the program's email address before 5:00 PM on April 29, 2026 (115). The Program Curriculum Committee will convene a second review meeting. Only those who pass this second review may apply for the proposal oral defense.

C. Not Approved: You must resubmit a brand-new application form.

Format of the Practicum/Internship Proposal Oral Defense:
The individual oral defense will be conducted by inviting your institutional advisor, your external internship supervisor, and one additional external examiner (to be decided by your advisor).
(Please submit your proposal to all committee members prior to the defense. When drafting the initial proposal, please pay strict attention to the formatting and ensure that both the Chinese and English titles are accurate. For the proposal layout, please refer to Appendix 3.)

Upon completing the proposal oral defense, students must submit the "Practicum/Internship Proposal Oral Defense Record Form" (Appendix 4) and the "Oral Defense Examination Fee Application Form" (Appendix 5) to the Degree Program Office (located at the College Office on the 1st floor).

Once your institutional advisor verifies that you have completed 100 hours of the internship, you must submit a "Midterm Report" (as shown in Appendix 6).
⚠️ Important Reminder for Appendix 5: For the Oral Defense Examination Fee receipt, please make sure to fill out the payee's information accurately, sign it, and attach a photocopy of the payee's bank passbook. For the external committee member's travel expense application, please also attach the original transportation ticket stubs from the day of the defense.
III. Important Deadlines

Internship Application Submission Deadline: From now until April 15, 2026 (115).

Internship Period: In principle, the internship should be carried out during the summer vacation. If necessary, the completion deadline may be advanced or extended upon receiving consent from both your institutional and external advisors.

Guidelines for Students Completing Internships and Seeking Credits
Please pay close attention to the following instructions if you have completed your internship and wish to obtain the 3 credits for [Practicum/Internship CEPH Requirements] in the 114-2 semester:
I. Document Submission
Upon completing your internship, you must fill out and submit the following documents before April 15, 2026 (115):

Practicum/Internship Evaluation Form - Internship Site Evaluation (Appendix 7)

Practicum/Internship Final Written Report (Appendix 8)

Request your institutional advisor to fill out the "Practicum/Internship Core Competency Evaluation Form" (Appendix 9)

Practicum/Internship Results and Deliverables Description (Appendix 10)

Two pieces of professional work deliverables/products (in any format)
⚠️ Critical Warnings:
Consistency: The project title (topic) and the names of the deliverables must be identical across all documents; otherwise, the application will fail the review.
Advisor Review: Please have your institutional advisor review all assignments/deliverables before uploading them to NTU Cool. The program office will download and archive your work at the end of each semester to provide to the Curriculum Committee for review.
Credit Acquisition: Please submit the aforementioned documents back to the Degree Program Office. Credits will only be awarded after being reviewed and approved by the Curriculum Committee.
II. Deliverables Review Results
The review results are categorized into: Approved, Revision Required for Re-evaluation, and Not Approved.

A. Approved: You are eligible to apply for the Public Health Practicum/Internship CEPH credits.

B. Revision Required for Re-evaluation: You must email the electronic file of your revised documents to the program's email address before 5:00 PM on April 29, 2026 (115). The Program Curriculum Committee will convene a second review meeting. Only those who pass this second review may proceed with their proposal oral defense or apply for the CEPH credits.

C. Not Approved: You must resubmit all relevant documents regarding your internship deliverables.

Key Focus of the Checklist: The names of "Deliverable 1" and "Deliverable 2" must be completely consistent across all documents.
